First, if the opening is ONCE UPON A TIME , you need to use past tense. You are correct. But for Q.1 'a boy' goes with a singular verb, so1. was
4. shared
5. kept
6. put {past tense form same as present tense form}
11. will see {a conditional statement: if the boy do the things his mother told him, he will.....}
12 did
The last two I'm not so sure. You can refer to ONCE UPON A TIME or NOW. Maybe I'll use present tense.
13. has
14 feels
the others are alright
