想看紅葉11月尾去大阪 will be better.
1.If you only have 5日行程, you better not go to 和歌山和有馬溫泉 because they both take quite a while to travel there.
2.If you are going to Osaka in November, you can still go to 神戶 - 香草布引園 for sightseeing and 神戸港 (if in November, there is Christmas tree and Christmas decorations)

4. I highly recommend 魚心壽司. If you don’t know how to order, ask them for English menu. They sometimes show a Set in the front door. Just tell them you want a SET will be ok (around yen 2000). The other good dishes are : Botan Ebi (shrimp), Tuna and 帶子壽司.

5. According to travel expert, 5日行程 (4 nights hotel), around $5500 CX flight, not including the Tax and extra charge(around $600-$700)
6. I will suggest “Osaka New Hankyu” 梅田新阪急 (not ANNEX).

DAY 1 - 機場 ->大阪站/梅田附近商場
DAY 2 - 環球片場 (Universal Studio)
DAY 3 - 京都–嵐山, 嵯峨野, 渡月橋, 嵐山, 清水寺，祇園, 金閣寺 [睇風景為主] [stay in京都 Hotel]
DAY 4 - 京都 -二条城-> 大阪 -> Late Afternoon : 難波/道頓堀/心齋橋一帶 [Optional : 美國村/南船場]
DAY 5 -大阪城 And / Or 大阪水上巴士 -> Rinku Premium Outlet -> 機場 (if your flight is 6:00pm)

If you want in Kobe, do not stay in 京都 Hotel on Day 3, stay in Osaka New Hankyu Hotel for all four nights.
DAY 4 - 神戶 - 香草布引園 And/ Or 神戸港 -> Late Afternoon : 難波/道頓堀/心齋橋一帶 [Optional : 美國村/南船場 ]

If you want to go to Nara,
Day 4 Morning : Go to 奈良 - 奈良公園, 東大寺、興福寺、春日大社
