serveral pieces of slightly firm and congested tissue together measure 6.2x5x2.5cm.The largest piece measures4.8x3.5x2.5cm.Cut surfaces of the largest piece show whitish tissue with focal congestion and a cavity is present inside which is irregular in shape and measuring 2.8x2.5x1cm.No content is present inside the cystic cavity. No grittiness is present on sectioning. (A)Largest piece,1 tissue block.(B)Other pieces,3tissue blocks.Figure(1)shows the submitted specimen.Figure(2)shows cut surfave of the largest piece.
Sections show fibrous thickening of the pleural tissue entrapping irregular foci of inflammation. These are mostly granulomatous lesion with irregular areas ofcaseous necrosis.Epithelioid histiocyes and occasional multinucleated giant cells are observed. These are features in keeping with residual foci of partly treated tuberculosis.Tfibrosis extends into subpleural lung parenchyma where there are fibrous thickening of alveolar wall, prominance of pneumocytes and bronchial metaplasia of repair. They entrap some thick walled vessels. Some of the alveoli contain detached aggregates of histiocyes.
there is no evidence of neoplastic proliferation.Malignancy is not seen.In the lung parenchyma proper, caseous necrosis is not present .Ziehl-Neelsen stain will be performed for assessment of mycobacterium and a supplementary report will be issued.
