有無人知道係什麼情形下要做 LONG SERVICE FUNDS PROVISION,
是否當員工服務滿了5年, 就要在會計帳做這個 PROVISION????

Yes. As a matter of prudence, a Provision for Long Service is to be made. The basis of such provision is in Q.5http://www.labour.gov.hk/faq/cap57l_whole.htm#Q6
