我諗住係4月14日請考試假同5月10-14日同屋企人去旅行，但係封請假信唔知點寫。唔該幫幫忙俾個我英文請假信格式我呀...thanks

I am writing this letter to request two period of annual leave. I would like to take a one day leave at April 14 for my examintaion. Another period will be taken from May 10 to May 14.
