Nope, when I type it I really am laughing. That's the first thing I get, if I've talked to someone from here on messenger and then talk to them on the phone, "you really are a giggly girl aren't you." Always! =)
I put lol for the hell of it and LOL if I really am. I save LMAO for those times at work when I have to suppress real, out-loud laughter or risk looking like a lunatic.
I use it as an instruction for the reader. It's like stage directions in a play. (at this point, you laugh, not just a small chuckle, but a nice healthy guffaw, so that anyone listening would say "she's laughing out loud!")
