For question1, I use "decide" to explain the difference between the present perfect tense and past perfect tense.
1. had decided 是past perfect tense,當我們正在說一些過去了的事情時,而要表達兩件事前後發生的次序,才會用past perfect tense

例如:After I had decided to visit Mary, my mother told me that I had to stay at home to look after my sisters. visit mary 係先發生的事情,而mother told me...是較後發生的事情

2. decided 是past tense,之前已說過是有明確時間表明已過去的事情
例如:Yesterday, i decided to visit Mary in the hospital.

3. have /has decided是present perfect tense,之前已說過是沒有明確的時間,只說明現在的狀態

例如:i have decided to live in *****in since 1995./ i have decided to live in *****in for 9years.意即聿1995年起,我已決定住在沙田。

1) Because "might" is a modal verb. After a modal verb, the verb that follows should be infinitive, the original form.
In present perfect tense, "have" should be used.
only in past perfect tense, meaning sth happened before another, "had" would be used.

2) the "do" in the sentence is to emphasize the meaning. This sentence is probably used when someone question whether she really has a sister.
