完整申請表，特選滑動從EDB，與學生股份單一起應該到達學校總務處從2008年1月3日，到2008年1月29日。在股份單應該附寄以下項目：
A.證明/學院證書
B.證明/獎影印件
C.1個印郵票的寫明發信人地址的信封
