我沒有format，但有些嘢想提一提：1. 此文件請記得要以舊公司信紙出信，
因為人家不會認識你的新公司
2. 文件務必包含新+舊公司的各info
公司名稱、地址、電話、傳真、聯絡人
蓋章及簽名
3. 此更名啟示的生效日期+通融有效期
4. 新舊文件／賬務交接之處理，包括Vendor+Customer
訂單
發票
5. 若可行／有需要，請連同新公司之商業登記一併附件
