如何寫公司改名啟示???

我沒有format，但有些嘢想提一提：

1. 此文件請記得要以舊公司信紙出信，
　因為人家不會認識你的新公司

2. 文件務必包含新+舊公司的各info
　公司名稱、地址、電話、傳真、聯絡人
　蓋章及簽名

3. 此更名啟示的生效日期+通融有效期

4. 新舊文件／賬務交接之處理，包括Vendor+Customer
　訂單
　發票

5. 若可行／有需要，請連同新公司之商業登記一併附件

Dear Sir,

Please be informed that our Company will move to "a new address" with effect from "a date" under the new business name of "the new name" while others remain unchanged.
