I would suggest u "Grammar in Use" which is written by Martin Hewings.
also this is Cambridge University press. This book is particular for self-study
the better thing is it has different level you can choose one which suit you.
it has reference and practice book for different learners of english...
I have been using it to improve my english as well even I am studyingin England

however, about listening and speaking, you have to try ur best to speak more and listen more...
I do think BBC is really good to improve listening..so...DO go to bbc.co.uk to have a look

read more,speak more,listen more,write more...
read more from the newspaper...if you want to search something,just go to
english website first...
speak more...if you are brave...go to the sightseeing points,meet some
foreigners and chat with them...
listen more...listen to english music,watch english TV news,watch english tv shows...listen to the english radio...
write more...if you take notes,write those in english...
every time,you have something need to write it down...think it in english first...
