It really depends on the case u're using, or forms ur fillin in....
Generally names are sperated in 3 groups
First Name: Siu Ming / Tom
Last Name: Chan
Middle Name: This should be the complicated one... Western people usually have a name other than the name which they use on a daily basis. Eg. Mary Smith's full name can be Mary JANE Smith and JANE is her middle name....
But I guess in ur case, Tom is not a middle name. So write ur name as
Siu Ming Tom Chan
