I don't know all the answers to your questions, but as far as I know you (the exchange student) will have to provide money for your own medical care and tuitions, the host family will provide you room and board.
Do you know which state you are going to? The East and the West coast are usually cost more, I lived in the Midwest and the living standard here is very low, I would say you will need about $100 -$200 to live on each month; of course you can always get a job at a local restaurant to get some spending money. High school in the U.S is free but books aren't.
If you want to go to university, you will need high scores on the TOFEL, I don’t know how TOFEL works as I am a citizen and don’t need this test.
