Cleaning problem for rabbit

When I help to cut finger nail for my rabbit, I find her " pet pet" quite
dirty. So I help to clean with water but I find there are some "meat ball"
outside the ass..are there disease? Need to see doctor? My rabbit is female
and around 4 years old. Thanks for your all reply first!
