MEASLES
MUMPS
RUBELLA
VARICELLA (CHICKEN POX)
History of disease?[ ] Yes[ ] No
OR Varicella Vaccine (two doses, one month apart, are recommended if the first is given AFTER the 13th birthday. If given before the 13th birthday, one dose is acceptable.)
HEPATITIS (THREE DOSES REQUIRED)
MENOMMUNE VACCINATION

你可以拎張野去任何一個私家醫生度幫你搞掂如果你是大學生﹐拎去校醫度就可以
一般你都會有 polio + MMR + TD
varicella 就睇你以前有沒有出過（水痘）
Hepatitis 多數是指 Hep B ﹐多數都應該打左
menommune 就多數都沒有﹐但依家應該好多地方有得打啦
所以不用頭痛~~
