2. Who will accompany you? (The Italy trip is going to happen in the future. Future tense is appropriate.)

3. Where is she going? (Ms Lai isn't at her desk at this moment, it's present tense. So the question shall also be present tense - to be exact, it's present continuous.)

4. What happened to him. (Tom had black eye when you saw him, so the thing had already happened. Appropriately to use past tense here.)

5. Were you waiting for someone? (It's past tense when I 'saw' you, so shall use past continuous to enquire whether at that time you 'were waiting' for someone.)
