Hi,
You will be better off taking coach from London victoria to Derby. If your flight arrive at 5am, you can take underground from Heathrow to GreenPark, then change victoria line to Victoria, arrive by 7:45am. Then take 08:30am coach from Victoria coach station and arrive derby at 12:10pm. (3hr 40 mins) If you book online now

I suggest not to fly to Manchester because it will take you minimum 2 hr 40 mins to derby + waiting time at Heathrow and flight time to Manchester.

Will anyone meet you from the Manchester airport? If someone gives you a lift from Manchester, then I will wait for the transit. It all depends how much luggage you have... For myself, I usually go straight to London and get a train directly to my locations, say London St Pancras to Derby, only with not too much luggage.
