when should i add m'/s'/t'appelle(s)/(z)
and why sometime is appelez..sometime is appelles or applelle...

m' [me], s' [se] and t' [te] are personal pronouns and the endings to the verb [in this case, appeler] has to be changed according to who performs the action and when the action is performed. In grammatical terms, this is called 'conjugation' - similar to English saying I/we/you/they go, he/she/it goes but a bit more change in French.

Je m'appelle Jean = I call myself John or I am called John [French way of saying 'My name is John' ]
Tu t'appelles Jean = You call yourself John or You are called John ['Tu' is an informal/familiar way of addressing the second person]
Il/Elle s'appelle Michel/Michelle = He/She calls himself/herself Michael/Michelle or He/She is called Michael/Michelle
Vous vous appelez Jean = You call youself John [Vous is in formal way of saying you. Use it when you don't know the person, the person is older/more senior than you or in general business communication]

But the verb appeler is a tricky word which everybody has to learn at the very beginning, as it can be used to tell someone your name or to telephone somebody, etc and one doesn't need to use the personal pronouns in every case.
