How to use forget

I want to know if i use "yesterday i forget to leave down your phone number"&"yesterday i forgot to leave down your phone number".Which one is correct.

THE SECOND ONE：yesterday i forgot to leave down your phone number.IS CORRECT!!
BECAUSE it happened yesterday.It is PAST!
SO you should use simple past tense.
Forgot is the past tense of Forget.
So you should use forgot in this situation.
The first one is wrong.
