(urgent) How to write an absent letter for my daughter ?

How is she? Is she OK now? I suggest a letter to the class teacher as below for your reference.

i think u just write when did your daughter get fever, add your signature, and hand in the letter with the doctor paper to prove your daughter really got a fever
