Sick leave application

Dear (Name of your teacher),

My son/daughter (your name) is absent from school on (the date you are absence). He/She is suffering from (the disease e.g. high fever, flu, cold,headache....). We have consult our family doctor in the afternoon. From the doctor's advice, he/she has to stay home and take a break from school for a day. For details, please see the attached doctor's note.

I know that we've better to phone to school on that day in the morning but due to some personal matter, I've forgot to do that. So I am now writing to explain why (your name) is being absent on that date.

Please accept my appologize and approve his/her delayed sick leave application.
