This sentence can be rewrite as: investigating officer said a investigation into whether the braking system was faulty was underway.When we say sth is/gets underway, we mean sth is being done. The word investigation ( as a noun) or investigate( as a verb) must always go with the preposition "into" followed by the subject of such investigation.
