降血壓藥?

Q1. It seems to be pretty new. Its patent under Servier in France is still effective and the clinical trial for hypertension was done in 2004. (Check company webpg for more info: http://www.servier.com/pro/cardiologie/coversyl/coversyl_press.asp) Since the patent is still effective, it would be more expensive then those that have a generic version. (e.g. Lisinopril, Peridopirl, Quinapril....)

初 次 服 用 此 藥 ， 可 能 令 血 壓 會 急 速 下 降 ， 病 人 可 於 睡 前 服 用 此 藥 ， 以 防 止 頭 昏 眩 暈 ， 此 情 況 會 慢 慢 適 應 。 孕 婦 ， 或 計 劃 生 育 的 病 人 不 能 服 用 此 藥 。

規格：每錠含Perindopril salified with tertbutylamine 4 mg (相當於Perindopril 3.338mg)。

適應症：高血壓、充血性心衰竭。

禁忌：禁用於
1.已知對本品或其他ACE inhibitors有過敏反應者。
2.曾使用其他ACE inhibitors導致血管性水腫者。
3.遺傳性或自發性血管性水腫者。
4.孕婦(懷孕第2至3期)者。

作用機轉：Acertil®是一種angiotensin converting enzyme (ACE) inhibitors，使angiotension I無法轉變成angiotension II (vasoconstrictor)，並減少aldosterone之分泌，降低鈉、水滯留而達降低血壓的效果；同時使bradykinin (vasodilator)不會被代謝成不活性的heptapeptide，故可防止血管的過度收縮。

藥物動力學：Perindopril為前驅藥，可代謝成活性代謝物perindoprilat，口服約65-70%會被吸收，與血漿蛋白結合率約為60%，其trough/peak ratio 75-100%，作用時間可長達24小時；與食物併用會降低活性代謝物約35%的吸收，主要經由肝臟代謝，約有75%由尿液排出，perindopril降壓效果產生於投藥後1.5小時。

用法及用量：高血壓：每天1次，每次4-8 mg，最大建議劑量為每天16mg。
充血性心衰竭：每天1次，每次2-4mg。

副作用：常見有頭痛(23%)、咳嗽(12%)、眩暈(8%)、虛弱(8%)、背痛(6%)等。
其他不良反應包括：水腫、胸痛、胃腸道障礙、神經肌肉方面的不適症狀。

藥品特色：
1.T/P ratio 75-100%，為同類藥最高，顯示其降壓效果平穩並且每日投藥一次即可。
2.EUROPA及PROGRESS study證實Acertil®對low risk CAD病患及stroke、TIA患
者有治療益處，包括：CV death、MI發生率相對降低20% (NNT=53)、low risk CAD
病患相對降低total mortality 14% (NNT= 43)、stroke復發率相對降低28%
(NNT= 27)。
3.於治療post MI及心衰竭，產生first dose hypotension比例較同類藥低。
