協議書
本人XXX女士於2006年12月30日惠顧xxx美容中心之〈眼部護理療程〉，並於2007年1月3日全數支付有關費用共$1,500。
本人並同意有關〈眼部護理療程〉的爭議和責任一概與xxx美容中心無關，並承諾不會再就【眼部護理療程】向xxx美容中心追究任何責任及賠償。

UNDERTAKINGI, XXX, signed up an eye treatment course at XXX Beauty Centre on 30th December, 2006 and duly paid the course fee totalled HK$1,500 in full on 3rd January, 2007.
I hereby agree that XXX Beauty Centre is not liable whatsoever to any dispute relating to the eye treatment course and undertake that I will not initiate any actions against and/or seek any indemnity from XXX Beauty Centre.

I hereby declare that xxx Beauty Clinic is not liable for the termination of the course and all responsibilities thereafter.
I also agree not to pursue against xxx Beauty Clinic any further for liability and compensations.
