若只是針對你發問的那部份：句一在省除了逗號之後就可以
句二在意思上有問題，因為must notify和use form是兩個並立句子，沒有甚麼關係，你亦沒有說要用特定的表格去報上新地址。
在文法上:
notify company漏了個article/possessive adjective。你該寫作：notify the company, notify our company...company form ABC是很怪的一個寫法。如果這是公司內的告示，寫上form ABC而行了。
