Follow the steps below to accomplish your job:
A. In Excel
Store the data to be printed in a database format. Well-organized and properly filled with appropriate data.
Let us say you have an Excel file with the following header in the 1st row

B
C
D
E
F
1
Salutation
First Name
Last Name
Address 1
Address 2
Address 3
2
Then properly fill this worksheet with data starting Row #2
After finishing with your data, save your file, exit.
B. In Word2003
B01. Click [Tools] [Envelope & Label] [Merge]

B02. Then the Merge Dialogue Box will then appear at the right side of the screen. Chosse [Label], [Next], [Next]

B03. Choose your label type, [OK]. Click [Next]

B04. Click [Browse] to select your data source

B05. In the [Data Source] Dialogue Box, Choose your Excel File previously created. Click [OK]

B07. Click [Insert] to add the data you want to shown on the label. After the selected, click [OK] to continue. Then arrange the data in its proper places on the label.

B08. Choose [Next] to preview the final look of your label
