1. As far as I know, the games are regionally locked, so that means US version consoles can only play US version games, while on the other hand, Japan version game will only be compatible with Japanese consoles. The menu language will be different of course.
2. Depends on how well you can read Japanese, for some of the games such as zelda, you cannot even play if you cannot read the japanese instructions (or hints); and you might have difficulty to use the menus if you do not know japanese at all. However, I have to admit that there will be more cool games coming up in the future, which you won't find in N. America at all.
3. Japanese electronics have the same voltage as in N. America, I have bought Japanese consoles in Japan and use in Canada, I am sure you do not need an adapter.
Myself I have already got a US version, I will buy a Japanese version next year in Japan. Make sure check out both nintendo sites of US and Japan:
