質量証明信一般由製造工廠自已寫翻，如果L/C無指明的話，用你受益人名字寫都得，不過最好由工廠寫。我現提供式樣給你，這一定ok的，是我日日用的。

CERTIFICATE OF QUALITY
We hereby certify that the quality of the shipment under L/C No. xxxxxxx was manufactured with best materials and first class workmanship. We further certify that all units being shiped have been inspected, tested and meet all of our factory quality Assurance standards.

至於產地來源証，一定要由在美國的註冊商會發出才有效，應該L/C都會寫 certificate of origin from chamber of commerce. 美國有統一的產地來源証，名稱叫: U.S. Certificate of Origin.
由各洲 recongized Chamber of Commerce 簽發。 例如我的貨品是由 The South Houston Chamber of Commerce in Texas 簽發。你叫你工廠出貨後去拿喇，你在香港做唔到的。你工廠一定懂做，上面不會有價錢，只有B/L no. 出貨人，貨運人資料等等，同海運的B/L 提單形式好似。

I am also in a trading firm. In general,

Certificate of Quality is issued by the L/C Beneficiary(you) to certificate the goods are in good quality). For example, if in LC it state " Certificate of Quality is required". Then YOU just use your letter form and type this setance + chop and sigined by authorized person.

Certificate of Origin usually issued by the Chamber of Commerce. I don't know why they ask for certification from you. If that is the case, you do the same things as in Certificate of Quality + chop and signed by authorized person.

***********Please note that make sure the L/C really calls for the above 2 certificates issued by YOU. Otherwise, there will be a discrepancies occurs.
