An ophthalmologist is certified to treat eyes diseases and does surgery, etc. An optometrist treats vision impairments.

They treat people with disorders that effect vision, such as glaucoma, diabetes, and macular degeneration. They often perform surgery on the eyes. They do not usually prescribe glasses like optometrists do.

Optometrists are Doctors of Optometry, and are trained to detect vision problems and prescribe glasses or contacts. For serious eye problems, they must refer to the guy above.
