終止保單的書面通知

終止保單的書面通知簡單得很，只需要列出
1：保單號碼，保戶姓名，終止日期，Policy Owner姓名及簽名便可。直接寄信到該保險公司的Policy Owner Service。
2：不一定需要該保險公司的終止保單表格，任務形式的書面通知均可，只要有以上資料便足夠。
3：行動前，請想清楚為何作此決定。
4：有否損失？
5：有另外選擇沒有？

Dear Sirs,
