終止保單的書面通知

如果我想終止我份保單,但個agent話要書面通知,請問應該點寫呢?

可唔可以比個sample我呀?唔該!

終止保單的書面通知簡單得很,只需要列出
1:保單號碼,保戶姓名,終止日期,Policy Owner姓名及簽名便可。直接寄信到該保險公司的Policy Owner Service。
2:不一定需要該保險公司的終止保單表格,任務形式的書面通知均可,只要有以上資料便足夠。
3:行動前,請想清楚為何作此決定。
4:有否損失?
5:有另外選擇沒有?
Sample of Ternination Letter

Policy Owner Services,
Address

Dear Sirs,
Re: Ternination of Policy No.XXXXXXXXXX
Name of Insured: xxxxxxxxxxxxxxxxxxxxxx

Please terninate the above policy with effect on xxxx-xxx-xx
on my own accord. Kindly return the residual value if any to my address as per policy record.
