怎樣寫一份公司要求員工寫的Performance Review的報告?

本人的工作是會計, 最近公司要求我地員工寫一份Performance Review的報告, 但進入這公司工作才兩個月, 但我真是無從入手, 問題如下:
1. List what you consider to be the key tasks for which you are responsible.
2. What have been your main achievements or sub performance in year 2006.
3. What are your goals in relation to the business in 2007? Itemize them.
4. What development or training would you like to help you to do even better in your job and/or to advance your career?

據實回應便可，但要運用非量化的語句：

1. List out -- key points -- you response
2. Write down you are on probation, that is.
3. Write down you would like to discuss with your supervisor end of the probation.
4. Write down which career training or course your prefer to attend, even you are studying or not?
5. Give response or comment to other questions with positive thinking.
