First off...where did you go and who with and what do they make or do there...next what sort of things you saw (we were show several offices, the canteen but not the experimental labs). This makes it plain which bits of the 'place' you will be talking about.
New paragraphs...Explain what things you noticed, room by room. Make sure you include any specific things the guide showed you or talked about in each room or area.
New Paragraph, holiday arrangement (if know); how do they cope with accidents; any specific jobs titles that are to do with the welfare of staff and / visitors and/ customers.
Last paragraph, include statistics about what they produce or do, any other information you have that's not include in other places, and round off by saying whether or not the visit was enjoyable and educational.

A visit report is basically a summary of what you did on a trip with an emphasis on lessons learned or recommendations for furure actions that could benefit your employer.
